My sons CRF 250 R intake valve clearence is a tight .004 but an easy .003 inch.
Would you shim the valve when it should be .005 inch ?

or is this still ok as the manuall recomends +- .001

Tight means it is not .004

1 shim size down gets you to .005 with maybe a little give based on your post. Still less than the .006 max.
